Register as a New Patient

The doctors welcome new patients who live within our practice area.  As it often takes some time for records to be forwarded from your former practice, all newly registered patients will be asked to complete a registration form and a health questionnaire. We will also ask you for proof of identity and address.

When being registered, patients have a GP they are assigned to and you can request to be with a GP you prefer.

You will then be offered a consultation with the healthcare assistant for New Patient Health Check.
