You can update information in your patient record online using Patient Access.

Through Patient Access you can update your phone number, home address and more.

If you need information from your medical records you can request them by visiting the surgery with your driving license/ passport during our opening hours. You will then be given an Access to Medical Records form to fill in and return to us.

To view your medical records online using Patient Access you can visit us during our opening hours with your driving license or passport to request access to your medical history.

We will provide you with access to you medical records from April 2020 to present day. For access to records before April 2020 you can visit us with your driving license or passport and we will give you an Access to Medical Records form to complete.

To access medical records of a family member, please click the link below. You will then be taken to a new page to fill out a form which must be brought to the surgery.
